We offer extremely competitive self-pay rates.  If you do not have insurance, we will still see you! 

Please call our office for a quote today.  

Additional Fees:

There is an additional $20 fee for completion of forms for use outside of the patient's medical record, including:

  • Family Medical Leave
  • Disability 
  • Insurance

Be sure to bring payment and the forms you need completed to our office, and allow 3-5 business days for completion.
